Advance Care Planning (ACP) is an ongoing conversation about what matters most if you are unable to speak for yourself due to a serious illness or health crisis. It involves reflecting on your values, beliefs, and healthcare preferences, and sharing early with your loved ones.
This hands-on workshop is designed for individuals who are in generally good health and wish to get a head start on drafting your own Advance Care Plan. Participants will gain an appreciation of the purpose and value of ACP. A live demonstration of an ACP conversation to support participants through their writing process. By the end of the workshop, participants will produce a first draft of their ACP with details on the next steps towards formal completion.
Each participant is encouraged to come with a spokesperson or next-of-kin, as ACP is fundamentally a shared conversation about future care preferences.

The main facilitator:
Ms Yeo Tan Tan is a certified Advance Care Planning (ACP) facilitator and trainer who has guided hundreds of ACP conversations with patients and their families in acute healthcare settings. She had trained clinicians in ACP facilitation and contributed to the advancement of ACP practice through her service on the National ACP Steering Committee.
She will be assisted by:

• Ms Yvonne Chen (Assisi Hospice Clinical Pastoral Care Counsellor)
